Although most people with COVID-19 improve within a few weeks of the illness, some people experience debilitating Post-COVID conditions long after infection. Post-COVID conditions are a wide range of new, returning, or ongoing health problems people can experience four or more weeks after first being infected with COVID-19.   • It begins with an interview. Your nurse practitioner will evaluate your symptoms, review your pre-existing conditions, and work with you to identify your recovery goals. Your nurse practitioner may order additional tests to assess your overall health better.


  • Your physical therapist will:
    • conduct endurance tests that will guide your initial activity recommendations
    • evaluate your gait, balance, capacity for exercise, pain, joint stiffness, flexibility, and range of motion
    • develop a strengthening and conditioning program based on your unique recovery goals and needs


  • Your mental health therapist will:
    • identify your recovery goals
    • screen you for anxiety, depression, and other mental health conditions
    • prescribe a program tailored to treat your symptoms


Patients enrolled in a Lumen medical plan will qualify if they:

  • provide documentation of a positive COVID-19 test and/or anecdotal evidence of COVID-19 exposure
  • exhibit any of the aforementioned symptoms for more than 12 weeks


While the program lasts 12 weeks, our support never ends.
